We have received numerous questions in regards to applications to Form 6. With the number of questions popping up on your feed and the variety of questions received, we decided to compile a comprehensive guide to Form 6 applications. So read on, and hopefully, any qualms you may have about applying will be quelled.
The main thing you have to understand is the different kinds of modes that exist for Form 6. There are 3 modes in total, the first mode is the Sixth Form College/Form 6 College, which refers to schools that have at least 12 classes that only houses Sixth Form pupils and are taught by teachers who have been appointed as Form Six teachers. There are only seven colleges across the country.

Accepts SPM graduates from SMK Jinjang, SMK Raja Ali, SMK Kepong Baharu & SMK Raja Abdullah.
Operating since: 10 April 2014.
Accepts SPM graduates from SMK Padang Serai, SMK Kulim, SMK Sultan Badlishah & SMK Tg Panglima Besar.
Operating since: 5 Feb 2014.
Accepts SPM graduates from SMK Bandaraya, SMK Inanam, SMK Kolombong and SMK Tebobon.
Operating since: 1 April 2014.
Accepts SPM graduates from SMK Dato’ Penggawa Barat and SMK Perhentian.
Mode 2 is a school that has at least 12 classes, operating in mainstream schools but in a separate block or building. However, the second mode is governed by the Principal of the school with help from the Sixth Form Senior Assistant.
Mode 3 is a school with less than 12 classes or schools that are far from the city, there are no boarding facilities or public transport, and operate in mainstream schools.

Students do not need to apply for sixth form because the offer is automatic. Revised bids can be made online through the system that will be opened soon. To candidates who do not get an offer, they can appeal at their respective State Education Department.
Here, we will talk about the application to appeal, and the 5 different kinds of appeals that can be made, along with the respective forms.
This form is meant for GOVERNMENT AID SCHOOL STUDENTS who want to make an appeal to Form Six otherwise offered may not meet academic requirements except Students with Special Needs (MBK). This form must be accompanied by copies of the documents that have been confirmed as follows:
i. SPM 2016 results
ii. Birth certificate,
iii. Identity card / passport
iv. School leaving certificate
v. Other related documents
Completed forms should be returned to the original school principal. The school principal is responsible for all pupils to parts of the different departments / JPN / PPDs concerned (except SMK (Harian) and SM Teknik/Vokasional). The application deadline is in May 2017. Late applications will not be considered. Consideration will only be given to applications that meet the following conditions:
i. A pass in SPM 2016 and acquired the predetermined academic requirements of and active in extra-curricular activities
ii. Obtained a credit in Bahasa Malaysia and two honors again in any other subjects
iii. Does not exceed the age limit stipulated in the general conditions
iv. Has the full support of the school principal and a certificate of good discipline record
Approval will depend also on the availability of places. Application results will be relayed to all applicants from the beginning of June 2017. If approved, the student must report within the timespan stipulated in the offer letter. If a student does not report within the prescribed period, the student is assumed to have rejected the offer.
The T6BCP form is reserved for PRIVATE Candidates, PRIVATE SCHOOLS & Sekolah Menengah Agama Negeri who apply to enter Form Six Year in 2017 in Fully government supported schools. Here are the documents that must be submitted together with copies of the documents that have been confirmed as follows:
i. A pass in SPM 2016 and obtained the minimum academic achievement that was set;
ii. Obtained a credit in Bahasa Melayu and two honors in any other subjects
Approval will depend also on the availability of places. The application results will be announced by the State Education Department (JPN) to all applicants through the School Sixth Form Center each or informed by the respective Divisions (except SMK (Harian) and SM Teknik/Vokasional) / Education Department of the State / District Education Office to all applicants by mail.
This form (T6BRTJ for GOVERNMENT AID SCHOOL STUDENTS) is meant for students who want to make an appeal to change their stream or department in Form 6 2017. Completed forms should be attached copies of the documents that have been confirmed as follows:
i. SPM Examination Results 2016
ii. Career Interest Inventory
iii. Related documents
This form must be returned to the State Department of Education (JPN) / Regional Education Office (PPD), with a form of acknowledgment from the school principal at the school the student was offered a place in, prior. The application deadline is in May 2017 (after the results of applications are released, appeal right away). Incomplete or late applications will not be considered.
However, approval of this application depends also on availability/openings and application results will be announced by the State Education Department to all candidates by the respective School Sixth Form Centres.

First, check carefully and make sure that the desired stream/department is offered at the school. Then complete the form P.U. (A) 275 below and attach supporting documents have been verified. (Reasonable grounds should be filled in the form to increase the chances of approval). After that meeting with the principal or teacher at the form 6 school you were offered a place in, you will need their signature of approval. The school will send your application form to the related District Education Office (PPD).
Bearing in mind, the key factor is that there is an exchange agreement void place at the requested school. Decision notification letter sent to the address of the applicant. Applicants may contact the PPD / JPN to know the status of the application after the last date of application.

The T6BRU form is for students to appeal to Form Six Year 2017 Sekolah Menengah Pendidikan Khas (SMPK) and Program Pendidikan Khas Integrasi (PPKI). This form must be accompanied by copies of the following documents that have been confirmed:

The application deadline is in May 2017 (after the results were announced, if not accept the offer continues to appeal), the application is not completed late and will not be considered. Consideration will only be given to applications that meet the following conditions:
i. A pass in SPM 2016 and acquired the academic achievement of predetermined
ii. Obtaining credit in Bahasa Melayu and two honors again in any other subject unless MBK manamata
iii. Do not exceed the age limit stipulated in the general conditions
iv. The full support of the school principal and a certificate of good discipline record
