Passed SPM/V with five (5) Credits, in Malay, English, Mathematics, one (1) Science subject (Biology / Physics / Chemistry / General Science / Applied Science), and one (1) other subject; OR Passed O-level with Grade C in English, Mathematics, one (1) Science subject (Biology / Physics / Chemistry / General Science / Applied Science), and two (2) other subjects; OR Passed Health Sciences Certificate (MQF Level 3) from MSU College, with 2.00 CGPA; OR Passed Malaysia Certificate (SKM) Level 3 in related fields and passed SPM with one (1) Credit; OR Passed Community College Certificate at MQF Level 3 in related fields and passed SPM with one (1) Credit; OR Passed Polytechnic Certificate (MQF Level 3) in related fields, with 2.00 CGPA; OR Passed MARA Skills Institute Certificate (MQF Level 3) in related fields, with 2.00 CGPA; or Passed Unified Examination Certificate (UEC) with Grade B in three (3) subjects; OR Other qualifications recognized as equivalent by the Government of Malaysia.
